On May 28, 2025, at 12:15PM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced at ABC Childcare Center (CCC) and met with Assistant Director (AD) Shannon Mouser and Administrator (AD) Mari Rivera to discuss the investigative findings regarding the allegations listed above. As part of the complaint process, LPA conducted two separate visits: the first on April 16, 2025, at 3:38 PM, and a follow-up visit on April 23, 2025, at 8:20 AM. During the second visit, LPA made observations at the facility, took a census, conducted confidential interviews with three staff members (S1–S3), and requested relevant documentation.

The investigation was initiated following a complaint received on April 11, 2025, alleging that a staff member had made inappropriate comments to children. Specifically, it was reported that Staff 4 (S4) was heard saying, “Don’t make me pop you.” However, interviews with three staff members did not corroborate the allegation that S4 made inappropriate comments as a form of discipline. All three staff members confirmed that, once management was made aware of the concerning remarks, immediate action was taken to address the situation.

A second allegation claimed that a staff member told another staff to pull a child’s hair. The complaint stated that because Child 1 (C1) was not napping, S4 instructed Staff 3 (S3) to pull C1's hair. LPA attempted to interview all three individuals involved in this reported incident. However, only one interview was completed, as the other two individuals declined to participate. The single completed interview could not corroborate or recall whether S4 made the alleged statement. It is also noted that the original complaint included the acknowledgment that there were no witnesses to the alleged actions by S4.

Due to conflicting statements and a lack of sufficient evidence, LPA was unable to corroborate the allegations that a staff member made inappropriate comments to children or instructed another staff member to pull a child’s hair. Although these incidents may have occurred, there is not a preponderance of evidence to prove or disprove the allegations. Therefore, the allegations are determined to be unsubstantiated.

An exit interview was conducted, and a copy of this report, along with the appeal rights and a Notice of Site Visit, was provided to Assistant Director, Shannon Mouser and Administrator, Mari Rivera. The Notice of Site Visit must remain posted in a prominent location, visible to families and caregivers, for 30 consecutive days.
