25 | On January 26, 2023 at 11:30AM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced to the facility to conduct a case management visit due to an unusual incident report received for an incident that occurred on 1/12/24. The incident involved two children being left in the classroom hallway, inside the facility doors. LPA met with Director Chantal Uhrich and conducted a tour of the facility. Appropriate ratio's and supervision was observed.
During the visit, LPA conducted interviews with three staff (S1-S3) all who were involved with the incident. Interviews revealed that staff were in ratio at the time of the incident and S2 observed S3 leave two children in the hallway after entering the classroom. S2 immediately brought it to the attention of S3 and S3 retrieved the two children (C1-C2) who did not enter the classroom after a name to face transition in less then a minute.
LPA has determined that the facility staff took the necessary steps to immediately retrieve the children from the hallway and no children were left outside on the playground. Additionally, facility notified the parents and provided one on one training to all staff, to improve the efficiency of name to face policies. Based on the information gathered, there appears to be no violations of Title 22 Regulations found at this time, and therefore, there were no deficiencies cited during this inspection.
An exit interview was conducted, a copy of this report, LIC 811 (Confidential Names List) and appeal rights were reviewed with and provided to Director Chantel Uhrich. A notice of site visit was also provided and must remain posted for 30 days.
