On April 16, 2025 at 01:51pm, Licensing Program Analyst (LPA), Hayley McCarthy arrived at La Petite Academy.Inc to deliver the investigative findings of the allegation listed above. LPA met with Director, Suriati Cafaro and Assistant Director Jacquelyn Negrete.

On March 6, 2025 at 09:45am, Licensing Program Analyst’s (LPAs), Hayley McCarthy and Jesse Gardner initiated an investigation at the facility. LPAs met with Assistant Director, Jacquelyn Negrete and conducted a tour of the facility. 93 children were present along with 7 staff. During the investigation, LPA interviewed all pertinent parties, conducted record reviews, and made observations.

On March 3, 2025, a complaint was received alleging staff do not change day care children in a timely manner resulting in day care children developing diaper rash. LPA's conducted interviews with 8 witnesses. 6 of 8 witnesses stated there have not been issues with diaper rashes. Child 1 (C1)’s authorized representative stated that C1 is susceptible to diaper rashes that lead to bleeding and therefore does not fault the facility for diaper rashes. LPA conducted record reviews and observed that C1’s diaper was changed 3 times during the 9 hours C1 was in care on the day in question. LPA also reviewed an incident report that was provided to the authorized representative informing of the bleeding diaper rash.

Based on LPA’s observations, record review and interviews which were conducted the allegation that staff do not change day care children in a timely manner resulting in day care children developing diaper rash is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Appeal rights were issued and discussed with licensee and their signature on this form acknowledges receipt of these rights.

Exit interview was conducted and report was reviewed by Director, Suriati Cafaro and A. A notice of site visit was given to director and must remain posted on, or immediately adjacent to the interior side of the main door for 30 days. The report must be made available to the public for three years. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
