25 | Licensing Program Analyst (LPA) Ana Noble arrived at the facility for the purpose of verifying, Plan of Corrections for Preschool Program (this is a combination center which also has Infant and School Programs which were also toured #336300576 & #336300577). LPA met with Center Director Chamali De Silva and Nam De Silva, Applicant, toured the entire facility which also includes Infant and School Age.
The following items are still needing to be complete prior to licensing:
1. Repair broken/non-operable drinking fountain faucets, add toilet paper and paper towels to all classrooms.
2. Replace stained toilets seats and place liner in all cabinets.
3. Additional indoor activities are needed to meet the requested capacity of 12 children.
4. Install all window screens.
6. Set up outdoor shade structure.
7. Will review pictures of the outdoor slide with Licensing Program Manager.
8. Kitchen needs to be cleared, cleaned and set up for operation of the kitchen. Food items should be in the pantry, refrigerator needs to be in it location, plugged in and operable, sink and stove cleaned.
Licensee will send pictures or a inspection will be conducted if needed to verify all corrections have been completed.
Exit interview conducted and reports were reviewed and provided to Mr/Mrs. De Silva. All reports must be maintained for 3 years. |
