25 | On December 4, 2023 at 8:56 am Licensing Program Analyst (LPA) Jessica Rubio conducted a Case Management inspection to follow-up on the compliance plan. LPA met with Director Leslie Marquez and Licensee Nam De Silva.
LPA toured the facility and took a census of the children in care. LPA first observed of total of seven children in two classes. Room Two had four children with one teacher providing care and supervision and Room One had three children with one teacher providing care and supervision. While LPA was present a staff for the preschool class arrived with two additional children for Room Two and one additional child arrived in Room One. The arrival of the additional two children in Room Two made the ratio in Room Two 1:6. LPA advised Licensee Nam De Silva of the classroom being out of ratio and referred Licensee to Title 22 Regulation Section 101417. Licensee had enough staff to have an additional staff go into Room Two to assist and immediately resolved the issue. In addition, LPA reviewed the Director and four staff files to ensure the staff met minimum qualifications for the positions in which they were hired. LPA requested additional documentation for recently hired Director by December 16, 2023.
During today's tour of the facility, other than the ratio issue that was immediately resolved, there was no evidence of any violation of regulations. No deficiencies were cited.
Exit interview conducted and report was reviewed with and provided to Licensee Nam De Silva. A notice of site visit was given and must remain posted for 30 days in location visible to all parents entering the facility. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
