NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On the above noted date and time, Licensing Program Analyst's Diana Brasel and Griselda Castellon conducted an announced Pre-Licensing inspection for a new infant component, currently the facility has a preschool license, which capacity will decrease. This facility will become a single licensed facility for preschool and infant. Upon arrival, LPA met with applicant/licensee America Figueroa. Applicant is requesting to be licensed for (12) infants 0 - 24 months of age in the infant room and 7 preschool children ages 2 through 5 years of age in the preschool room. Hours of operation will be Monday - Friday 6:30 - 5:30 pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA's informed America Figueroa that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, America Figueroa stated that they will be in the kitchen A fully equipped first aid kit is located in each infant room and preschool room. There is an operational carbon monoxide detector on site located in each room, infant room and preschool room. All required licensing documents were observed posted in the office/reception area. Children will be signed in and out at the office/reception area. LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for infant is 420.72, which is sufficient to accommodate the requested capacity of 12 children. Indoor space for preschool is 260.06, which is sufficient to accommodate the requested capacity of 7 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. -continued on LIC 809C-

Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the via filtered bottle water, disposable cups, sippy cups, and children's water bottles. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property.

Fire clearance was granted on 07/24/24.

High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. The current changing table does not meet required regulations. Cribs meet regulatory requirements, there are a total of 6 cribs in the napping area. The crib area is separated from activity space via a wooden partition wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the napping area attached to each crib. Needs and Services Plans and Sleep Plans are kept in the infant room.

There is at least one potty chair or toilet for every five infants. LPA observed a total of 1 sink available for children’s use. These are sufficient to accommodate the requested capacity of 12 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the office/reception area.

Facility will provide breakfast, lunch and pm snack. Meals are delivered by catering service. The kitchen area currently includes a refrigerator, microwave, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a locked door.

The facility currently has a fully fenced playground area which is separate from other components. Fencing chain link and building block wall. The total square footage for all the infant outdoor activity space is 370.74, which is not sufficient to accommodate the requested capacity of 12 children.

A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade will be added to the infant playground (see correction needed). There are sufficient outdoor age-appropriate toys and play equipment available on the playground. The total square footage for all outdoor activity space for the preschool is 708.75, which is sufficient to accommodate the requested capacity of 7 children. Drinking water is available via children's water bottles and igloo. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant America Figueroa was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Criminal Record Clearance - Child Care Centers:

Applicant/Licensee was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant currently has an approved IMS plan on file. This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. padding covered with moisture-resistant, washable material. Sides of the changing table are at
least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for
meal preparation or dishwashing)

2. Proof that the front bay window located in the infant room that has metal with sharp areas and
splintered chipped wood has be repaired or made inaccessible.
3. Proof that all holes on the walls in the infant room have been repaired shall be submitted.
4. Proof that shade has been added to the infant playground and that the playground has been
extended as discussed at time of pre-licensing visit. A return visit will be required to re-measure
once proof of correction has been received.
5. The applicant shalll submit a waiver and provide a schedule to share the infant playground.

The return remeasure will include the infant and preschool playground.

An approved granted waiver for the infant playground time schedule shall be obtained.

Applicant/Licensee America Figueroa understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

A notice of site visit was given to applicant/licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. “
