At 11:35 a.m. on Friday, July 14th, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Meghan Mara, for the purpose of an unannounced, case management - incident inspection. On 6/9/2023, the facility self reported an incident that occurred on 6/7/2023 regarding an injury sustained on the play structure. A report was received and is on file with the department. During today's inspection, LPA made observations to the cushioning beneath the play structure. LPA also discussed annual fees.

No citations were issued during today's inspection. An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
