13 | At 9:00am on 2/18/2026, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility to continue and deliver findings to a complaint investigation into the above allegation. Upon arrival, LPA observed a census 12 school aged children supervised by 1 staff.
Throughout the course of the investigation, LPA conducted observation, interview, and record review relevant to the allegation that staff did not provide supervision adequate to meet the needs of a child, resulting in the child sustaining an injury during play. Interviews with three individuals present at the time of injury provided corroborating evidence that a teacher was standing approximately 10 feet away from the play activity and observed the child when they engaged in the quick action that led to injury. Evidence that could establish that child's individual needs were not met by the quality of such supervision was inconclusive. Based on evidence gathered, the allegation is found to be UNSUBSTANTIATED, meaning that although the allegation may have occurred or is valid, there is not a preponderance of evidence to prove it.
