25 | Licensing Program Analyst (LPA) Kelly Ferrara conducted a Plan of Correction inspection at the facility and met with Director Jody Darone. On September 29th, 2021, LPA observed 47 preschool children in care with four staff.
On September 3rd, 2021, LPA issued the following citation: 101216.3(b) due to a classroom having one teacher and one aide with no units supervising 20 children. LPA received an updated staff schedule showing that the aide is placed in a classroom with a smaller group of children. Aide was not working during today's inspection. LPA was able to clear the citation and provide letter as proof.
On September 3rd, 2021, LPA issued the following citations: 101216(g)(1) and Health and Safety Code 1596.8662(4)(b)(1) due to staff files not having a completed LIC503 or Mandated Reporter training for some of the staff. During today's inspection, LPA observed that those staff had a current Health Screen on file. LPA was able to clear the citation and provide letter as proof. However LPA observed that three out of six staff were still missing Mandated Reporter training. Civil penalties for failure to correct were assessed.
On September 3rd, 2021, LPA issued the following citation: 101221(a) Children's Records are separate and complete. LPA had observed that children's files were missing forms LIC701 and CDPH286. During today's inspection, LPA observed that the children missing the LIC701 forms and proof of immunization still do not have completed files. Civil penalties for failure to correct were assessed.
There were no deficiencies cited based on today's inspection. LPA provided a Notice of Site visit that must remain posted for 30 days for parental review. |
