25 | On August 27, 2025, Licensing Program Analyst (LPA) Soleil Marx met with Director, Dawn Allred, for the purpose of conducting a Case Management Inspection regarding a self reported incident. The purpose of today's inspection was explained.
Sacramento RO received an Unusual Incident Report from the facility self-reporting an absence of supervision incident that occurred on 08/20/2025. Facility was verified in compliance with reporting requirements. During today's inspection, LPA made observations, reviewed documentation, and interviewed Director. LPA determined by incident report, investigation supporting documents, and interview, that two children were left outside unattended in the play yard from approximately 9:15 am to 10:00am, when their class transitioned into the classroom from the outdoor activity space without them. A staff member of another class found the children outside playing approximately 45 minutes after their class had gone inside. A health check was conducted, parents were notified, and the staff member responsible for the incident has received disciplinary action in the form of termination. Director stated following the incident, transitions are monitored by management oversight and staff training will be conducted on supervision, attendance counts, and transition procedures. Based on Today’s inspection, One Type A Title 22 deficiency is being cited on 809-D and an immediate civil penalty of $500 is being assessed.
Director was informed that this report dated 08/27/2025 documents one Type A citation which shall be posted for 30 consecutive days. The Director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted and a copy of this report was given to the Director. Notice of site was provided and must remain posted for 30 days. Appeal rights provided.
