32 | However, based on review of administration records and staff statements, LPA determined that the staff did not provide the second afternoon dose of the medication for at least the recorded days of 1/21/26-1/30/2026. According to staff, this was due to a belief that the medication was an extended release, all-day dose. The preponderance of evidence standard has been met; therefore, the allegation is SUBSTANTIATED.
(2) Medications are not stored appropriately
It was alleged that staff did not appropriately store medication. Through staff interviews, LPA determined that, on 3/18/2026, C1's medication was not found in the central location designated to store medications, which is a lock box in the kitchen. According to staff, the medication was later found that day in the nearby kitchen refrigerator. The preponderance of evidence standard has been met; therefore, the allegation is SUBSTANTIATED.

LPA Matthew Gallo informed facility representative Dawna Allred that this report dated 3/26/2026 documents 2 Type A citations which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Gallo informed the facility representative to provide a copy of this licensing report dated 3/26/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the facility representative, Dawna Allred. A notice of site visit was given and must be posted for 30 days. LPA provided appeal rights.
