13 | An unannounced subsequent complaint investigation inspection of this infant facility was conducted by Licensing Program Analyst Jennifer Velasco (LPA), who met with Facility Representative, Director Parveen Sandhu (D1). During today’s inspection, LPA toured the facility, conducted interviews, reviewed facility documents, and observed five classroom staff providing care to 15 infants. It has been alleged the facility is operating out of ratio. During the investigation, LPA toured the facility, observed care to children, conducted interviews, and requested and reviewed pertinent documents. Witness statements, document reviews, and LPA observations failed to corroborate the allegation that the facility was operating out of ratio. The preponderance of evidence standard has not been met; therefore, the above allegation is found to be UNSUBSTANTIATED. This report was reviewed and discussed with D1. Appeal Rights were provided and exit interview was conducted. All licensing reports are public information and must be made available upon request for at least three years. D1 was provided with a Notice of Site Visit (NOS). The NOS is to be posted for parental review in the facility for 30 days.
