Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with Director (D), Parveen Sandhu, to deliver findings. Throughout the course of the investigation, LPA toured the facility, observed staff providing care to children, reviewed documentation, and conducted interviews. LPA observation, record reviews, and interviews revealed that staff present, were not qualified and did not have the appropriate documentation on file to meet Title 22 requirements. LPA interviews and witness statements revealed that staff do not monitor food intake or special diets of infants as they were given and offered restricted food, and infants received bruises and bite marks without staff knowledge. LPA observation, record reviews, interviews, and witness statements corroborated with the allegations Staff are operating out of ratio, Staff do not follow infant's feeding plan, and Infant(s) received unexplained injuries. The preponderance of evidence standard has been met, and the allegations are SUBSTANTIATED.

Type A Title 22 Deficiencies are issued on the attached LIC9099-D page. Director shall provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. Exit interview was conducted with Director, appeal rights were provided, and A notice of site visit was given to Director, who will post it where visible to parents/guardians for 30 days. A signed Acknowledgement of Receipt of the Licensing Report (LIC 9224) must be placed in the child's file for verification.

LPA interviews, statements, and record reviews were inconsistent to corroborate the allegations Staff is not providing healthful accommodations to children in care, Staff do not report injuries of infant(s) to authorized representative, and Staff are not keeping facility free of odor. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

Exit interview was conducted and report was reviewed with Director (D), Parveen Sandhu. Appeal rights were provided, and a Notice of Site visit was given to Director, who will post it where visible to parents/guardians for 30 days.
