25 | On December 20, 2022 LPA Lea Habtom conducted a case management visit in response to the unusual incident report received on December 14, 2022. The director, Mary Patten, self-reported the incident to licensing on form LIC 624 unusual incident report. Today's census was 43 preschool children and 11 toddlers being supervised by 10 staff members in the Kinder, Safari, Turtle, Armadillo and Barnyard classrooms. All individuals present today have fingerprint clearances and are associated to the facility.
LPA L. Habtom conducted an interview with the director Mary Patten who reported the incident. On December 13, 2022 staff member (S1) was in the Safari classroom getting ready to transition the 10 children outside around 10:30 am. S1 had the children line up in the classroom and began calling the children's name's. As the teacher was escorting the children outside the teacher failed to follow policy procedure by having the children line up against the wall outside of the classroom prior to exiting the facility. At that time the teacher was to do a face to name check for each child and mark on the facility roster that each child was accounted for during the transition. The teacher then failed to perform a name to face check at the wall prior to entering the playground. The teacher took 9 children outside to the playground not realizing one was in the classroom. Around 10:40 am the cook (S2) was delivering food to the Safari classroom for lunch and noticed a child in the classroom alone playing in the kitchen area with no staff. Management was notified by the cook and the child was brought out to S1. The director contacted the parents and notified them of the incident.
Based on the interview with the director and the self-reported unusual incident report LPA L. Habtom has determined that there was a violation in Title 22 regulations which has been cited on 809-D.
Title 22 Deficiency has been cited on the attached LIC 809-D. LPA Lea Habtom informed the director Mary Patten that this report dated December 20, 2022 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the director to provide a copy of this licensing report dated December 20, 2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights given. |
