13 | Licensing Program Analyst (LPA) Erwina Pascual-Golamco and LPA Jennifer Velasco met with Facility Representative, Director Sandra Haines (Director), for the purpose of conducting an unannounced initial complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained to Director. During today's inspection, LPAs conducted interviews, observed care, and obtained relevant documentation.
Witness statements, LPA observations, and/or document reviews failed to corroborate the allegations. Though witness statements and documentation reviews showed that a child suffered an adverse event while in care, witness statements and documentation showed facility staff attended to the child in a timely manner, did not observe more severe symptoms unitl parent had arrived, and did notify parent child was ill. Although the allegations may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Sandra Haines. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
