On 03/27/2024, Licensing Program Analysts (LPA) Jennie Tedlos and Licensing Program Manager (LPM) Karyn Guerra conducted a case management inspection at the facility. LPAs met with Substitute Director, Alejandra Paredes. At time of arrival LPA observed 49 children and 8 staff.

During the facility inspection, staff files were reviewed. LPA did not observe updated personnel files showing the required educational documentation for staff which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and file review, the center's Director has been on leave since late January 2024. The facility did not obtain a fully qualified substitute director in the absence of the center's Director which poses a potential health, safety or personal rights risk to persons in care.

Two Title 22 deficiencies have been cited on subsequent page, LIC 809D.

An exit interview was conducted, and a Plan of Correction was reviewed and developed with the Substitute Director, Alejandra Paredes. A copy of this report and appeal rights were discussed and left with Substitute Director. A Notice of Site Visit was posted by LPA’s and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
