32 | It was alleged that Staff did not inform child's authorized person when the child was injured. There was concern of an incident in which a child had a mark that was not reported to the child's authorized person. Interviews with Staff revealed conflicting information. Through record review, LPA observed a documented incident report signed by a parent. Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED.
Based on the information obtained throughout the course of this investigation the above allegations, LPA Tedlos determined that the allegations were found to be UNSUBSTANTIATED, meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
Exit interview was conducted. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
