Licensing Program Analyst (LPA) Jennie Tedlos met with Substitute Director (SD), Alejandra Paredes to deliver findings of the complaint investigation regarding the above (2) allegations. There 60 children and 6 staff during the visit. Throughout the investigation, LPA interviewed parents, staff, and children, and conducted record review.

It was alleged that staff caused injury to a child in care. Interviews with SD and staff revealed that an incident happened where a teacher (T1) caused a red mark on a child's (Child 1) nose after pinching it. LPA also reviewed documentation of the incident in Child 1's file. Although a mark was left, interviews revealed that this incident did not cause injury to Child 1. T1 is no longer employed with the facility. Children's interviews revealed consistent information. Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED.

It was alleged that Staff did not inform child's authorized person when the child was injured. There was concern of an incident in which a child had a mark that was not reported to the child's authorized person. Interviews with Staff revealed conflicting information. Through record review, LPA observed a documented incident report signed by a parent. Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED.

Based on the information obtained throughout the course of this investigation the above allegations, LPA Tedlos determined that the allegations were found to be UNSUBSTANTIATED, meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.



Exit interview was conducted. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Licensing Program Analyst (LPA) Jennie Tedlos met with Substitute Director (SD), Alejandra Paredes to deliver findings of the complaint investigation regarding the above allegation. There 60 children and 6 staff during the visit. LPA Tedlos conducted an investigation regarding the complaint allegation listed above. LPA toured the facility, conducted interviews with the Reporting Party (RP), Staff Members, Children enrolled at the facility and parents of children that attend or who have attended the facility. LPA also obtained pertinent information to assist with the investigation.
It was alleged that a Teacher (T1) violated the personal rights of a child (Child 1) after making inappropriate comments to Child 1. It was revealed during Staff and Child interviews that T1 regularly yelled inappropriate comments to the children in care. T1 is no longer employed at the facility. Based on interviews and file reviews conducted the preponderance of evidence standard has been met. The above allegation is found to be SUBSTANTIATED. A Type-A Deficiency is cited on a subsequent 9099-D page.

SD states that T1 has been terminated from the facility as of 3/22/24. SD also states that the facility has conducted multiple personal rights trainings, including the CCLD Personal Rights Training Video to staff since the incident occurred.
