25 | At 3:30 p.m. on Thursday, August 18th, 2022, Licensing Program Analyst (LPA) Karyn Guerra met with Acting Director, April Vasquez for the purpose of a POC inspection. A citation was issued on Thursday, August 4th, 2022, regarding reporting requirements. During today's inspection, LPA conducted a file review and observed signed acknowledgement of receipt of licensing forms LIC9224 in children's files. The citation has been corrected per POC and is cleared.
During census conducted, LPA observed 10 preschool children in the Redwood classroom with 1 Staff (S1). LPA conducted a file review and interview and came to find that S1 is not a teacher. This poses an immediate risk to the health and safety of children in care and is a repeat violation. Civil penalties were assessed.
LPA also observed that the temperature in the Sequoia's classroom was elevated in comparison to other classrooms. LPA observed the thermostat reading 81 degrees. Director stated that maintenance was conducted earlier in the week but it is having issues. Technical assistance was conducted. LPA observed children moved from the Sequoia classroom into the cooler daycare classroom during inspection. It was stated that children will combine in the daycare classroom during the evenings. LPA advised for monitoring of children to ensure that they appear comfortable and are not overheating.
Title 22 deficiencies are cited on the subsequent pages of this report. Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Acting Director, April Vasquez. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days. |
