25 | On 06/09/2022, Licensing Program Analyst (LPA) Katy Maestas conducted a field visit to Center for the purpose of a Case Management inspection. LPA disclosed the purpose of the inspection and was granted entrance into the Center. LPA was greeted by Center Director Jessica Dawson-Brown (D1). LPA toured the Center and observed 14 children, under the age of 2 years, being supervised by 4 adults. LPA observed 47 preschool aged children under the supervision of 7 adults. LPA observed 11 school aged children under the supervision of 1 adult. LPA determined through Guardian that all required adults in the facility were background cleared.
LPA interviewed D1 regarding an Unusual Incident Report (UIR) that is dated 05/17/2022. LPA interviewed 2 staff members and reviewed staff files. LPA reviewed children's files but did not interview any children due to their young age. LPA requested a copy of the Facility Roster and a class list. Documents were collected by LPA regarding the UIR.
No deficiencies were cited today, 06/09/2022. An exit interview was conducted, and the report was reviewed with D1. Licensee Appeal Rights were provided to D1. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
