At 11:30 a.m. on Wednesday, October 19th, 2022, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Liliya Farfan, for the purpose of an unannounced complaint investigation. It was alleged that staff place burp clothes over infants' heads. At 11:47 a.m., LPA Guerra observed blankets covering the faces of 4 napping children in the infant classroom. LPA discussed with staff that infant's heads shall not be covered, and the blankets were immediately moved away from the face. It was also alleged that staff do not perform sleep checks. Interviews with staff stated that staff are constantly supervising and checking on napping infants, however, documentation of 15 minute checks was not observed. It was also alleged that changing tables are not properly sanitized. LPA observed cloth coverings on changing mats and a roll of disposable paper cover. While staff did state that cloth mats are sanitized after each use, LPA did observe dirt and

At 11:30 a.m. on Wednesday, October 19th, 2022, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Liliya Farfan, for the purpose of an unannounced complaint investigation. It was alleged that staff restrain infants in highchair. Interviews with Director stated that high chairs are used for lunch, snack, and for solo play at the consent of the child. It was stated that children will request up to play at the high chair and they will be given an activity to play. Director denied children being left in high chairs for prolonged periods of times or while children are upset. It was also alleged that staff serve foods that present a choking hazard to infants in care. Director stated that food served to infants is soaked or mashed as to prevent choking hazard. It was also stated that larger food may be served to infants so they can take a bite. It was also alleged that staff are not washing infant bottles. Director stated that milk or formula left in infant bottles

is sent home with parents at the end of the day, as not to waste. Director stated that once a bottle has been used, it is not given again to the child later in the day. It was also alleged that there is insufficient space between napping cribs. LPA observed crib area of the facility and interviewed staff. It was stated that some shorter staff may have to move a crib to reach an infant, but that infant children are not in the crib that is being moved. LPA advised about crib spacing. Changes were made to the crib space during inspection. The allegations are unsubstantiated. Although the alleged violations may have happened or are valid, the preponderance of evidence standard has not been met to fully prove of disprove that they did or did not occur, therefore, they are unsubstantiated. This report was reviewed with the director and a notice of site visit was provided and shall remain posted for a period of 30 days.

discoloring under changing mat. LPA did not observe a vinyl, wipeable surface. It was also alleged that infant sheets are not properly laundered. It was learned in an interview that infant sheets are laundered weekly, or as needed when soiled. The preponderance of evidence standard has been met, and the allegations are substantiated.

Title 22 deficiencies are cited on the subsequent pages of this report. Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days for parental review.
