13 | THIS IS AN AMENDED REPORT - On Tuesday, February 3, 2026, at approximately 12:30PM Licensing Program Analysts (LPA) Julia Maryanova met with Licensee, Joyce Pennybaker, to conduct an unannounced initial complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained. LPA observed 3 children being supervised by Licensee.
It was alleged that Licensee is not providing a safe environment for children in care. Throughout the course of the investigation, LPA conducted interviews and made observations. LPA observed that the facility was not maintained clean or sanitary condition. Clutter was observed blocking entryways, including stacked boxes that poses a rist of falling and causing injury to children in care. LPA observed the kitchen sink and counters filled with clutter. Based on observation the preponderance of evidence standard has been met and above allegations are substantiated.
Deficiencies are noted on subsequent page of this report LIC9099-D. An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days. |
