25 | At 12:15 p.m. on Friday, September 9th, 2022, Licensing Program Analyst (LPA) Karyn Guerra met with Licensee, Sima Eskandari, for the purpose of a required 1 year inspection. Census at time of arrival was 1 infant child under 12 months. Licensee's spouse/assistant was also present during time of inspection. Hours of operation are 7:30 a.m.-5:30 p.m., Monday thru Friday. Off limit areas are daughter's bedroom and living room.
All individuals subject to criminal background review have obtained a criminal record clearance. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A health and safety inspection was conducted in all areas accessible to children. LPA observed the required postings, a working phone, 2A10BC fire extinguisher, and functioning smoke and carbon monoxide detectors. Licensee stated that there are no weapons nor poisons in the home. Toxic and hazardous items were appropriately stored, inaccessible to children. Fireplace is appropriately barricaded to prevent access by children. Fire Drills have been conducted and documented.
LPA reviewed staff and children's files and observed current in person EMSA CPR and First Aid certification, expiring 4/4/2023. LPA observed immunization records, emergency identification, signed parents rights, affidavit regarding liability insurance, notification of additional children in care, and consent for emergency medical treatment. Individual infant sleeping plan was on file for infant child under 12 months. Napping logs were observed.
