Licensing Program Analyst Erwina Pascual-Golamco (LPA) and Licensing Program Manager Natalie Dunaway (LPM) met with Licensee Mercy Ishida to deliver findings of the above allegation.

Throughout the course of the investigation, LPA toured the facility, observed staff provide care to children, reviewed files, and conducted interview. It was alleged that Type A licensing reports were not provided to parents/authorized representatives for 09/20/23 and 07/24/24 incident dates. LPA reviewed sample of children's files on 10/09/24 visit. Out of 6 sample files, 3 had the signed LIC9224 for citation date 09/20/23 only, none for 07/24/24 and 3 files did not have any LIC9224 for verification. LPA's record review corroborated the allegation Type A licensing report/s not provided to parents/authorized representatives for 09/20/23 and 07/24/24 incident dates. Licensing Program Analyst Erwina Pascual-Golamco (LPA) and Licensing Program Manager Natalie Dunaway (LPM) met with Licensee Mercy Ishida to deliver findings of the above allegation.

Throughout the course of the investigation, LPA toured the facility, observed staff provide care to children, reviewed documentation, and conducted interviews. Staff and parent interviews revealed that as needed, they use substitutes from Child Care Careers (CCC) who have been background cleared, and parents are familiar with the same staff present at the facility.

LPA Interviews, observations, and record reviews failed to corroborate the allegation that uncleared adult present and/or working at the facility. Although the alleged violation may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that it did or did not occur, therefore, it is UNSUBSTANTIATED.

This requirement is not met as evidenced by:
based on record review, Licensee (L) did not comply with the section cited above, LPA reviewed sample of children's files on 10/09/24 visit. Out of 6 sample files, 3 had the signed LIC9224 for citation date 09/20/23 only, none for 07/24/24 and 3 files did not have any LIC9224 for verification, which poses a potential health, safety, or personal rights risk to persons in care.
