On December 6, 2023 at approximately 10:30 AM, Licensing Program Analyst (LPA) Josiah Gathing met with Director Cynthia Bunton to follow up on a self-reported unusual incident report. LPA conducted interviews and made observations and determined that no Licensing regulations were violated. As a result, no deficiencies were cited during today's inspection.

This report was reviewed and provided to the Director. A Notice of Site Visit was provided and shall remain posted in the facility for 30 days.
