Licensee Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Fire Clearance was approved for facility to operate in two city-operated classrooms and 12 additional campus classrooms. However, the fire clearance STD850 form states facility is approved for 200 total capacity for Transitional Kindergarten through 5th grade students. Because this campus operates and serves children up to 8th grade, licensing requirements for a capacity increase states that the STD850 form must accurately reflect ages of children in care.
Prior to issuing a license, the following conditions must be submitted:
1. A new STD850 form must be received by licensing which reflects correct ages of children in care
School classrooms have all received a fire clearance, and although the 4th R program facility currently operates in city owned portables W1 and W2, classrooms A4, C4, D2, D5, GYM, and School’s Multipurpose Room, they have the ability to operate in other campus classrooms with this fire clearance. Transition of classrooms must be approved by licensing if they occur in the future, and facility will inform licensing as soon as reasonably possible if those situations occur in the future.
Once corrections are made, Facility will be approved for School-Age component (200 Capacity) Serving the Ages of: Enrolled in Transitional Kindergarten to 8th Grade in city owned portables W1 and W2, classrooms A4, C4, D2, D5, GYM, and School’s Multipurpose Room.
Exit interview conducted and report was reviewed with the Licensee Representative.
