13 | Licensing Program Analyst (LPA) Michelle Perez met with Facility Representative Tiffany Haynes on November 8, 2023, at approximately 01:00PM to deliver findings for the above allegation. Upon arrival LPA took a census of 5 infant children with two staff.
Throughout the course of the investigation, LPA interviewed the Reporting Party, owner, Director, and staff at the facility, as well as toured the facility, made observations and obtained information to review. LPA found that a school aged child was alleged to have co-mingled with infants while in care at the facility. LPA also found that there was a staff member (S1), who had been asked to watch a school aged child, while in attendance, but declined to do so. While interviewing, LPA also found that S1, was not fond of an infant teacher (S2), and that a school aged child, was related to S2, therefore, S1 refused to watch school aged child. LPA also found the facility utilizes a daycare app, which allows staff to check in/out children for care. |
