On February 10, 2025, Licensing Program Analyst (LPA), Michelle Perez, arrived at approximately 12:45PM for a case management, legal visit. LPA met with Director Wanda Woods and owner/operator Courtney Williams. The purpose of the visit was to determine if the facility has remained compliant with their legal stipulation, which states revocation: Stayed with probation. LPA had previously conducted a visit on January 24, 2025, in which multiple part of the stipulation were found to be noncompliant. Today, 2/10/2025, LPA toured the facility with the stipulation to document corrections made and areas that remained noncompliant.
Upon arrival there were 3 infant children with 2 teachers.

LPA observed the following items that remained noncompliant with the stipulation, but were addressed;

2L- Respondents shall continue to maintain safety devices on each door that allow for ingress or egress to the facilities and shall maintain them in good working order. The device shall emit a noise sufficient to alert respondents' staff when a door to the outside of the facilities is being opened. LPA witnessed door alarms on each door of each facility classroom, with the exception of the exit door, in the preschool classroom, nearest to the kitchen, did not have an alarm installed when LPA arrived. LPA informed director Wanda Woods of the noncompliance, director corrected the noncompliance with the installation of a door alarm. LPA observed the front door alarm not operating upon arrival. LPA informed director of the noncompliance and the door alarm was adjusted to working condition.

LPA observed the following items in compliance with the stipulation:

2C- This stipulation shall be posted in a conspicuous place at the facilities for the duration of the probationary period. This was observed and verified by LPA that the stipulation is posted in each classroom of each facility.

2I- Respondents shall maintain a missing child policy and procedure and shall post it in each facility classroom. This was observed and verified by LPA the enlopment procedure, which documents
the procedures staff will take, when a child is missing.

2N- Respondents shall, within sixty (60) days of the adoption of this stipulation, complete two (2) hours of training with each staff member related to the allegations contained in the accusation and approved by licensing. LPA observed and verified two hours of "active supervision" training certificates, for each employed staff member, within their files, documenting they have taken a class that covers,
1. Care and supervision and 2. preventing children from leaving the facility unattended.

2O- For the duration of the probationary period, respondents shall inform all current and prospective parents of children in the facilities of the facilities' probationary licenses by providing to the parents a copy of this stipulation and the attached accusation. Parents shall acknowledge indicating they have received a copy of the stipulation and the accusation. This parental acknowledgment shall be maintained in the corresponding child's file and shall be made available to the department upon request. This was observed and verified by LPA through a review of children's files, documenting that each parent has reviewed and acknowledged the probationary stipulation. Further, the facility has scanned the stipulation into a PDF document, that is sent to new and incoming parents, through a link, when the parents register their children for enrollment. The facility also acknowledges the parents have reviewed the PDF stipulation through an online document signed by the parents.

LPA discussed this report with Wanda Woods and provided a notice of site visit, that will be posted for 30-days.

LPA reminded director, that frequent visits will be conducted to ensure the stipulation is adhered to.
