1. Care and supervision and 2. preventing children from leaving the facility unattended.
2O- For the duration of the probationary period, respondents shall inform all current and prospective parents of children in the facilities of the facilities' probationary licenses by providing to the parents a copy of this stipulation and the attached accusation. Parents shall acknowledge indicating they have received a copy of the stipulation and the accusation. This parental acknowledgment shall be maintained in the corresponding child's file and shall be made available to the department upon request. This was observed and verified by LPA through a review of children's files, documenting that each parent has reviewed and acknowledged the probationary stipulation. Further, the facility has scanned the stipulation into a PDF document, that is sent to new and incoming parents, through a link, when the parents register their children for enrollment. The facility also acknowledges the parents have reviewed the PDF stipulation through an online document signed by the parents.
LPA discussed this report with Wanda Woods and provided a notice of site visit, that will be posted for 30-days.
LPA reminded director, that frequent visits will be conducted to ensure the stipulation is adhered to. |
