On Tuesday, July 12, 2022, at approximately 1:00 PM, Licensing Program Analyst (LPA) Josiah Gathing, met with Licensee Soodabah Hassanzadah, to conduct a Case Management inspection for the purpose of increasing the maximum capacity from 8 to 14 children. During the inspection there was 1 child being supervised by Licensee. LPA conducted a health and safety inspection of all areas accessible to children. Off-limit areas will include: bedrooms 1, kitchen, back yard, and building B. Applicant understands that children may never enter these off-limit areas.

The Fire Safety Inspection Clearance was granted on 07/06/2022. CPR/First Aid certificate is current and expires: 01/2023. The facility is equipped with a functioning pull alarm. Toxic and hazardous items are inaccessible to children and out of children’s reach. Sharp knives and children medication are stored in a kitchen cabinet above the refrigerator out of children’s reach. A functioning smoke detector, carbon monoxide detector, and 2A10BC fire extinguisher were observed. Applicant stated that there are no weapons in the home. The required postings were observed during today's inspection.

Applicant has completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training. Current EMSA approved pediatric CPR/First Aid training was verified and expires 2/6/23. Current Mandated Reporter training due was verified and expires 2/17/23.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

In the areas that were evaluated, no deficiencies were observed at the time of the inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Licensee, Hassanzadah Soodabah.



Capacity increase is approved as of today, 07/12/2022. The facility is licensed to serve a MAX. CAP (WHEN THERE IS AN ASSISTANT PRESENT): 12 - NO MORE THAN 4 INFANTS. CAP 14 - NO MORE THAN 3 INFANTS. 1 CHILD IN KINDERGARTEN OR ELEMENTARY SCHOOL AND 1 CHILD AT LEAST AGE 6.
