LPA observed multiple classes go out to the large playground and did not observe any children approach the empty garden boxes. On 06/17/2025, LPA observed the garden boxes had been removed. LPA observed documentation that they had been donated. LPA reviewed injury/ouch reports and did not observe any injuries caused by access to the empty garden boxes. The empty garden boxes did not appear to pose a hazard to children in care.
Witness statements, LPA observations, and document reviews failed to corroborate the allegation that staff did not provide adequate meals to children in care. Witness statements regarding meals provided to children were consistently positive and did not support the allegation that meals were inadequate. On 05/02/2025 and 06/17/2025, LPA observed the posted menu and observed staff prepare and serve meals to children. The meals served aligned with the posted menu. LPA observed leftover foods and milk from multiple classrooms. LPA also observed when children asked for more food, it was readily provided to them. There were witness statements that on one morning, the staff did not offer milk to children until breakfast was already underway. Though this was unfortunate, the children were offered the milk during the meal. Despite this single slight delay in children being offered milk, there is not a preponderance of evidence to support the allegation.
Although the allegations may have happened, there is not a preponderance of evidence to prove the allegations; therefore, the allegations are unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Michelle Ingram. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days. |
