25 | On August 11, 2023, Licensing Program Analyst (LPA) Erwina Pascual-Golamco, met with Director Emily Bell (D1), for a case management inspection regarding an incident that happened on 8/9/23. Census included 42 preschool children supervised by 7 staff. Purpose of the inspection was explained.
Director filled out a written Unusual Incident Report (LIC624), at time of LPA visit. On 8/9/23, a child wandered off the facility and LPA asked D1 why it was not reported to the department the next working day, in addition to the written LIC624, and D1 stated they informed facility representative about incident and that facility representative was going to take care of the reporting. D1 stated facility representative was not aware of the next working day requirement and only know about the 7 day written requirement. D1 also stated she should have reported it herself to make sure. D1 was not aware that it was not reported to the department until LPA informed D1 today. During today's inspection, LPA observed the play yard and interviewed D1. D1 stated that moving forward, they will comply with reporting requirements. D1 handed the LIC624 to LPA today. Deficiency cleared at time of visit.
Title 22 Deficiency has been cited for reporting requirements. Exit interview was conducted, appeal rights were provided, and A notice of site visit was given to D1, who will post it where visible to parents/guardians for 30 days.
