Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with Director, Emily Bell, to deliver findings. LPA toured the facility, including all activity and classroom spaces, and outdoor play areas. Director was reminded never to exceed the conditions, limitations, and capacity specified on the license.

It was alleged that staff does not maintain the facility in a clean and sanitary condition, staff fed a day care child the wrong milk, staff inappropriately speaks to day care children, and staff did not adequately supervise a day care child out on the playground. Throughout the course of the investigation, LPA toured the facility, observed staff provide care to children, reviewed documentation, and conducted interviews. LPA interviews, witness statements, and documentation revealed that aside from the twice weekly janitorial service of the facility, every classroom have daily and weekly checklist of responsibilities, staff clean and sanitize tables, chairs, and toys in between settings. There is a list of food allergies in the kitchen and is posted in every classroom that is updated regularly. Staff use redirection, speak at child's level, and problem solve. Children are also offered a cozy area. Staff are spread out in the classrooms and playground to cover all areas, and follow a 1:8 ratio according to Director. Written reports or phone calls are given to parent/guardians when a child sustains an injury while in care. Client interviews revealed that aside from occasional mess after meals, they do not have concerns with the facility's cleanliness and sanitation, nor have concerns with staff giving their child the wrong milk. Client observations and interviews revealed that children like their teachers, an adequate amount of staff supervise the children in the classroom and playground, and are notified of incidents/injuries. Interview statements, LPA observation, and facility documentation failed to corroborate the allegation that staff does not maintain the facility in a clean and sanitary condition, staff fed a day care child the wrong milk, staff inappropriately speaks to day care children, and staff did not adequately supervise a day care child out on the playground. Although the alleged violation may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that it did or did not occur, therefore, it is UNSUBSTANTIATED.

This report was reviewed with Director, and an exit interview was conducted. Appeal Rights were provided and a Notice of Site Visit (NOS) was provided to Director, who will post it for a period of 30 days for parental review.
