NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Application Specialist (AS) Mai Lor met with Applicant Francesca Winn for the purpose of an announced pre-licensing inspection. Present during this inspection are Co-Applicant Isobel Whitbread-Cole, staff members Shelby Parker, Sharee Russell, Selina Martinez, and Courtney Charriere. Applicant is requesting a preschool license to serve 84 preschool children from age two to entry into kindergarten. The program will operate Monday through Friday from 8:00 AM to 5:00 PM. The fire clearance was granted on 01/23/2023. AS reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Entrance Checklist was provided to the applicant. INDOOR ACTIVITY SPACE: The preschool has four classrooms (Lavender (Two Years Old), Rosemary (Two Years Old), Maple (3-4 Years Old), and Pine (5 Years Old)), a kitchen to support food preparation, laundry room, and office area. AS observed sufficient amount of age-appropriate equipment, toys, tables, benches, napping mats, and individual cubbies. There is a first aid kit in each classroom. Applicant stated medications will stored in each of the classrooms. Applicant stated there are no firearms or weapon on the premises. Applicant stated indoor and outdoor drinking water will be provided to children by tap water in the children’s individual water bottles. AS observed carbon monoxide detectors throughout the facility. Applicant stated the facility will be utilizing an electronic sign in and sign out system. Applicant stated they will be using electronic sign in and sign out system. Applicant stated paper sign in and sign out will also be available as a back-up. Applicant stated the facility will provide morning and afternoon snack. Parents will be responsible for lunch. AS measured the classrooms. The total classroom space contains a total of 2,981.69 square feet. which will accommodate the requested capacity for 84 preschool children. There are two restrooms equipped with four toilets and four sinks in each restroom. There is an additional restroom in the Lavender room with one toilet and one sink. Thus, there are 9 toilets and 9 sinks in the facility. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the staff office and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There is an outdoor activity space for preschool children. The outdoor play area is surrounded by a wood fence that is at least four feet tall. No play structure or climbing structure were observed. Applicant planned on purchasing additional outdoor equipment. There is a shade structure. Applicant stated she planned on attaching swings to the shade structure. Shade structure, trees and overhangs will be utilized as shade.

As measured the outdoor activity space. The outdoor play area contains a total of 11,025.38 square feet, which will accommodate Applicant's request for 84 preschool children.

AS discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. AS discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The applicant has requested for waiver to have one director manage the operation of two day care centers provided that the centers are adjoined through a connecting play yard.

Effective today, 1/27/23, AS grants a preschool childcare license to serve 84 preschool children age two years to entry into kindergarten.
