13 | Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with current Director Danielle Law to deliver findings of the above allegations. LPA toured the facility, including all activity and classroom spaces, restrooms, and outdoor play areas. Director was reminded never to exceed the conditions, limitations, and capacity specified on the license.
It was alleged that teachers not qualified and hazards accessible to children at this infant facility. Throughout the course of investigation, LPA conducted observations, reviewed records, documentation, and interviewed staff. On 03/20/24 LPA visit, it was determined by observation, record review, and interviews that 2 out of 3 staff were aides and 1 fully qualified infant teacher. LPA observed both aides were left inside the facility with 7 awake infants and engaged in activities, and the fully qualified infant teacher was in the playground with 4 infants and did not have visual observation on the aides. LPA interviews and documentation revealed that hazards accessible to children when disinfecting wipes are left on the table where the infants eat, and are stored where it is accessible to children. Based on LPA observations, interviews, documentation, and record reviews, the preponderance of evidence standard has been met, therefore the above allegations are SUBSTANTIATED.
Title 22 deficiencies are cited on LIC 9099-D. Exit interview was conducted and report was reviewed with Director, Danielle Law. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days. |
