Licensing Program Analyst (LPA) Loraine Perez met with applicants Zakera Hashemi & Wali Hassany for a Change of Location inspection. LPA observed 0 children in care during the inspection. Off limits areas include garage, primary bed and bathroom, bedrooms 2, 3, and 5, bathroom 2 and laundry room. This is a two story, five bedroom, three bath home.

LPA observed all areas of the home. LPA observed no bodies of water on the property. Toxins and dangerous items were found inaccessible to children. LPA observed stairs are barricaded to meet regulation.

Applicants stated no weapons in the home. Zakera Hashemi CPR/First Aid certification expires 03/2026 and Wali Hassany CPR/First Aid certification expires 11/2024. Applicants will complete Mandated Reporter Training and submit certificates of completion by email to LPA L. Perez. This training requirement may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: HTTP://WWW.MANDATEDREPORTERCA.COM/. Applicant understands both CPR training and Mandated Reporter Training must be completed every two years.

All adults in the home have criminal record clearances.



In addition, LPA tested the smoke alarm, carbon monoxide detectors and fire extinguisher meet Title 22 regulations.

LPA reviewed licensing forms and provided the licensing website, www.ccld.ca.gov, so applicant may download forms, or review regulations.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.


On this date, 09/09/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The applicant provided proof of control of property.



Pending both applicants completion of Mandated Reporter Training LPA will grant a large family child care license to serve 12 children (when there is an assistant present) with no more than 4 infants or capacity of 14 children when 1 child in kindergarten or elementary school and 1 child at least age 6 and a maximum of 3 infants. Infants are children under the age of 2 years old.
