Licensing Program Analyst (LPA) Loraine Perez met with Licensee Farahnaz Bayat for a Change of Location inspection. Licensee was previously licensed to facility number 343624332. Licensee understands that effective today, the old facility will be closed, and care can no longer take place at that location. Licensee's spouse was present during today's inspection and provided translation assistance in Licensee's preferred language. LPA observed two children in care during the inspection, both children are the licensee's children. Facilities hours of operation are Monday through Friday 07:00 AM to 05:00 PM.

This is a two story house, four bedroom, three bath home with kitchen, living room, garage and fully fenced backyard. Off limits areas include entire second floor, and garage. Applicant’s CPR/First Aid certification expires 02/2026. Mandated Reporter Training is not offered in licensees primary language, therefore she is exempt from the training. All adults in the home have criminal record clearances.

LPA observed all areas of the home. Licensee stated there are no weapons or bodies of water on the premises. Toxins and dangerous items were found inaccessible. Licensee stated fireplace is electric and is barricaded and will remain inaccessible to children. In addition, LPA tested the combination smoke and carbon monoxide detector. LPA observed the stairs to be barricaded to meet Title 22 regulations. LPA observed the fire extinguisher is a 2A10BC classification and meets Title 22 regulations.

LPA discussed capacity and supervision with Licensee. Licensee understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. LPA discussed Type A/B citations, open door policy, fire drills, children’s personal rights with the Licensee. Licensee understands a current roster of children enrolled must be available and maintained for a period of three years, even after children are no longer attending.

Licensee understands that if any structural changes are made to the home, licensing must be notified prior to construction. Licensee understands that if they want to make any off-limit area an on-limits area, licensing must be notified and LPA must do an inspection before children are allowed in the area. Licensee understand that licenses are not transferable, and once licensed, licensees must live in the home and be present for 80% of the operating hours.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensee provided proof of control of property.

LPA reviewed with Licensee the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the Licensee.

LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 12/20/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Licensee was informed of the https://MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Effective today, 01/06/2025, License is apprived for a Small Family Child Care Home.


This facility is approved for a small family child care home license to serve a capacity of 6 children with no more than 3 infants or 4 infants only. Or with a capacity of 8 children: no more than 2 infants, 1 child in transitional kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
