1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On August 21, 2025, Licensing Program Analysts (LPAs) Kyrsten Williams and Josiah Gathing met with applicants Hazura Singh Bal and Rajvinder Kaur Bal for the purpose of an announced pre-licensing change of ownership inspection. Present in the facility were previous owners, now directors, Simon Gill and Jasbir Gill. This is a change of ownership from Circle of Friends facility #343607981 and #343617513. Applicants are requesting a license to serve 28 preschool children ages 2 years to entry into the first grade and 8 infants ages 6 weeks to 2 years old. The total requested capacity is 36 children. Census included 5 infants being supervised by 2 staff and 9 preschool age children being supervised by 1 staff. The program will operate Monday through Friday from 6:00 AM to 6:00 PM. A fire safety clearance was granted on 06/23/2025. INDOOR ACTIVITY SPACE: All activity space utilized for the children was inspected today. There are a total of six classroom spaces which included two upstairs preschool classrooms, relaxation room, downstairs preschool classroom, preschool nap room, and infant classroom. LPAs observed sufficient age-appropriate equipment, toys, tables, chairs, mats and individual cubbies. There is a kitchen to support food preparation. Applicants stated that the facility provides breakfast, morning snack, lunch, and afternoon snack. Uncontaminated drinking water is readily available to children both indoors and outdoors via water fountains and cups. LPAs observed functioning carbon monoxide detector located in the downstairs preschool classroom, fire extinguisher located in the kitchen, and first aid equipment located in the kitchen. Director stated medications will be stored in the kitchen and inaccessible to children in care. LPAs observed an paper sign in and sign out sheets. There is a working telephone at the site. All toilets and hand washing facilities are in working condition with proper sanitation in place. Bins for solid waste have tight fitting lids. All the cleaning solutions, poisons and other chemicals that are dangerous to the children are stored inaccessible to the children. *PAGE 1. REPORT CONTINUES ON LIC809-C*

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The indoor classroom space was measured. The total indoor space for preschool age children contains a total of 1068.44 square feet, which will accommodate the applicant’s request of 28 preschool children. The total indoor space for infants contains a total of 291.48 square feet, which will accommodate the applicant’s request of 8 infants. There are three toilets and three sinks available to children. There is an additional bathroom with one full-size toilet and sink. There are two changing table for infants that is within arm’s reach of a sink. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the relaxation room.

OUTDOOR ACTIVITY SPACE:
There is an outdoor play area for preschool children and a separate outdoor play area for infants. The play space in the back of the center is surrounded by a chain link fence that is at least four feet tall. When facing away from the building, the left side of the play yard will be used for infants and has a square footage of 775.675, which is sufficient space for the requested capacity of 8 infants. The right side of the play yard will be used for preschool children and has a square footage of 3621.15, which is sufficient space for the requested capacity of 28 preschool children. Play structures do not have labels with manufacturer guidelines. Applicants acknowledge staff must ensure children use age-appropriate equipment at all times. All playground equipment and surfaces were inspected and are in good condition, with enough resilient material under climbing structures to absorb a fall. There are shaded areas for the children provided by trees. Applicants were reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicants acknowledge that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule.

LPAs discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Applicants and directors were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicants were informed of the www.MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Applicants were encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request.
