On 05/25/2023, Licensing Program Analyst Katy Maestas (LPA) arrived at the facility to conduct an unannounced Plan of Correction (POC) Inspection. Upon arrival LPA met with Director Christine Mendoza (D1).

During an Annual Random Inspection conducted on 05/17/2023, D1 was issued a deficiency for not having complete staff files with required documentation and immunizations. A POC was created that allowed D1 one week to update and complete all current staff files. On 05/25/2023, LPA inspected all current staff files. LPA provided D1 with a Clearance of Citation letter.

An exit interview was conducted and the report was reviewed with D1. LPA provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
