On 2/28/2023 at 10:40 am, Licensing Program Analyst (LPA) Patricia Berry conducted a case management incident investigation. LPA met with Malika Binns/Site Supervisor and was granted access into the facility. LPA toured facility and took a census. LPA conducted interviews with staff and obtained documentation.

On 2/15/2023 it was self- reported by facility that parent is alleging staff inappropriately handled the child.


Due to additional information needed, LPA will return at a later date to conclude the investigation.

Exit interview conducted with Malika Binns/Site Supervisor report, appeal rights and notice of site visit issued.

Notice of Site Visit must be posted for 30 days.
