On 3/9/2023 at 12:15 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent case management incident investigation to deliver the final report. LPA met with director and was granted access into the facility. LPA toured facility and took a census. LPA obtained documents.


On 2/15/2023 it was self- reported by facility that parent is alleging staff inappropriately handled a child. LPA conducted interviews with all pertinent parties. During staff interviews the staff stated they have not handled a child in a rough manner. Staff stated they have not seen bruises on any child. Children interviews indicated that staff do not handle them roughly or their friends. Children stated they like their teachers.

Based on staff and children interviews conducted It has been determined at this time there has been no violation of Title 22 regulations.

Exit interview conducted with site supervisor, report, appeal rights and notice of site visit issued.

Notice of site visit must be posted for 30 days.
