25 | On 07/15/2024 at 9:00 AM, Licensing Program Analyst (LPA) Tiffanie Diep met with Licensee Janalee Bradley for the purpose of an unannounced Plan of Correction (POC) inspection. LPA was greeted by a staff member (S1) upon arrival as Licensee was not present at the time. LPA observed S1 supervising 11 children, five of whom were school age. At 9:15 AM, Licensee returned to the facility.
All individuals subject to a criminal record review have obtained a criminal record clearance. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of five days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
During an annual inspection conducted on 06/05/2024, Licensee was issued a Type B citation as Personal Rights (LIC 613A) forms were not signed for two out of four children present. Licensee had agreed to submit copies of the completed LIC 613A forms for the two children. The POC due date was agreed upon for 07/05/2024; however, the POC has not been received as of 07/15/2024.
Licensee did not correct the previously cited deficiency, which results in a civil penalty of $100 per day for a total of $1,000. The civil penalty of $100 per day will continue to be assessed until the violation is corrected.
An exit interview was conducted and report was reviewed with the licensee, Janalee Bradley. A notice of site visit was given to Licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
