On March 6, 2025, Licensing Program Analyst (LPA) Kris Diaz conducted an unannounced Annual/Random inspection at the facility. LPA met with Director Annette Franquez who granted access to the facility and guided LPA on a tour the facility. LPA verified all staff have background clearance and TB test on file. LPA also verified that all staff have immunization against measles (MMR), pertussis (TDAP), and either immunization against influenza or a statement of declination on file. At the time of the visit LPA observed 82 children in care with 18 staff members. This facility has 5 classrooms that are being utilized for day care (A2, A3, B1, C1, C2). Class B2 is closed due to unavailable staff. Classrooms A3 and C1 are designated for children 3-5 years old. And classrooms A2, B1, B2, and C2 are designated toddler classrooms. This facility operates from Monday through Friday 7:45 AM - 3:15 PM. There is one part-day classroom that operates from 8:00 AM – 11:00 AM and 11:45 AM – 2:45 PM.

Per Director Annette Franquez there are no bodies of water on the premises and there are no firearms or weapons stored on the premises. All disinfectants, cleaning solutions, poisons and other items are locked in cabinets in each classroom inaccessible to children. All cabinets and drawers in each classroom are kept locked.

The facility utilizes the Child and Adult Care Food Program (CACFP). The facility provides morning and afternoon snacks for the part-day program and breakfast, lunch, and PM snack for full-day program. Menus are posted in the office near the main entrance. Allergy lists are posted in each classroom and the kitchen. LPA observed an appropriate amount of food and snacks in the kitchen properly labeled and stored. Per Director, perishable foods are usually supplied to last weekly. LPA observed non-perishables to be properly stored and labeled away from chemicals and poisons. The facility was clean, orderly, and provides a safe space for children in care. Parents conduct sign-in/sign-out daily at the entrance of the center.

LPA observed an indicator arrow in the green, fully charged, and operational fire extinguisher (2A10BC) located on premises. This facility has a functioning carbon monoxide detector, smoke alarm and fire pull station in each classroom that meets statutory requirements. All storage containers for solid waste, including moveable bins, have tight fitted covers and are in good repair. There are fully stocked first-aid kits available (provided by the campus police) in each classroom inaccessible to children. Medications are kept in the office in a locked box and for medications that may require refrigeration there is a dedicated space in the kitchen refrigerator. Each classroom has a small refrigerator to keep medications as well that is inaccessible to children. There is a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Furniture and equipment were inspected for age appropriateness and are in good repair. Each classroom was inspected and are clean and safe. Heating, lighting, and ventilation are adequate. Cubbies were observed in each classroom for children to store their belongings. Each classroom has a water jug and cups available for drinking water and children bring water bottles from home. Both are used for outdoor play. Cots used for napping are maintained in a safe condition. Bedding and linens are stored in individual bags labeled with children's names and stored in their cubbies. Laundry is done weekly at the facility. LPA observed safety and sanitation log which is completed daily for indoor and outdoor spaces in each classroom.

Restrooms A2 and A3 – 3 toilets and 3 sinks, B1 and B2 – 3 toilets and 2 sinks with changing table, C1 and C2 – 3 toilets and 3 sinks.

Outdoor play equipment was inspected and observed to be in good repair. Equipment is labeled in shared play yard to indicate age-appropriateness. The outdoor activity space is in safe condition and free of hazards. Each space has storage space for outdoor toys to protect from weather. There is adequate shade for children in care including natural shade from trees and tarps. There is fencing around the perimeter of the play area is at least 4 feet high. There is artificial turf with a rubber matting underneath for cushioning material under climbing apparatus accessible to children.

There is a separate area for isolation and care of ill children in the director's office. There are mats available for each ill child. The facility was observed to be free of flies, other insects, and rodents.

1. The following items are prohibited by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.



3. Current Children’s Roster: Each childcare facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.

4. LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations, and quarterly updates can be accessed on the Childcare Licensing website at: www.ccld.ca.gov.

5. Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.

6. Designated Staff: The name of the childcare center Facility Representative or fully qualified teacher(s) designated to act in the Facility Representative's absence must be on file.

8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles or influenza declination

9. Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.



Director, Annette Franquez was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.



LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and discussed the Childcare Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
