25 | On 7/3/2024, Licensing Program Analyst (LPA) Andrea Pittman conducted a Case Management Legal/Non-Compliance Inspection at the facility and was met by the Facility Representative. The LPA disclosed the purpose of the inspection and was permitted entry by the Representative. The Director guided the LPA on a tour of the facility. Upon entry to the facility, the LPA observed 18 children in care and three staff providing care and supervision.
During the inspection, the Decision and Order – CDSS No. 1223340101 was provided, read, and explained in detail to the facility representative. LPA Pittman observed that the employee, Michele Parks, was not present at the facility and the Director attests that the employee, Michele Parks, has not been present at the facility since December 8th, 2023 and is no longer associated to the facility. Michele Parks is excluded from all care facilities licensed by the Child Care Licensing Division (CCLD), licensed by the Department, for a lifetime unless they successfully petition for reinstatement under Government Code Section 11522.
The California Department of Social Services (CDSS) has determined that Michele Parks is prohibited from employment in, presence in, and contact with clients of, any facility licensed by the Department, certified or approved by a licensed foster family agency, or any resource family home, and from holding the position of member of the board of directors, executive director, or officer of the licensee of any facility licensed by the Department, for the remainder of their life.
Effective Monday, July 1st, 2024, the Decision and Order for Michele Parks has excluded her presence in any aforementioned facility for life. The facility representative acknowledges the receipt of the Decision and Order provided today, Wednesday, July 3rd, 2024.
An exit interview was conducted, a copy of this Report, a Notice of Site visit, and Appeal rights were provided and discussed with the Facility Representative. |
