25 | Licensing Program Analyst (LPA) Rachel Zeron conducted an unannounced case management visit at the facility for the purpose of conducting an investigation pertaining to the facility's submission of an Unusual Incident Report (UIR) received by Licensing on 06/06/2024. The UIR outlined an incident that occurred on 06/06/2024 pertaining to a child putting play dough in their ear. LPA was granted entry to the facility and met with Facility Director, Charlene Bunnell-McAlister to discuss the incident.
Interviews revealed that the child put a small piece of play dough in their ear while the class was having centers. The child started complaining about their ear and when the teacher checked, a small piece of play dough was lodged in the ear. Parent was called to have the child picked up and a small Band-Aid was placed over the ear to prevent the child from sticking their finger in their ear. Child was picked up and taken to physician to have it removed. Child is currently on a behavioral plan and is non-verbal, at the time of the incident there were 2 teachers and 9 children in the classroom.
Based on the information obtained during this visit, there appears to be no violations of Title 22 Regulations at this time.
No deficiencies were cited during this inspection and a copy of this report was provided to Director, Charlene Bunnell-McAlister. Director understands a copy of this report shall be kept on record for three years and provided to the public upon request. A notice of site visit was provided and must remain posted for the next 30 days.
