On 8/9/2022 at 1:00 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a case management incident that occurred on 8/1/2022. LPA was granted access into the facility and met with director. LPA toured facility and took a census. There were no children present during the visit. Teacher in-service day.

On 8/1/2022 it was alleged a parent informed staff through an app the teachers use to communicate with the parents, that his child had blood on the underwear; parent stated another child inappropriately touched his child. LPA conducted interviews and obtained documentation. Director stated police were called and stated there was nothing they could investigate since this incident was child on child.

Due to insufficient information at this time, LPA will need to return at a later date to deliver the final report.


Exit interview conducted with director, report, appeal rights and Notice of Site visit issued.

Notice of Site Visit must be posted for 30 days.
