On 12/4/24 at 2:38 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent case management incident investigation in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 11/7/24. LPA observed no children present during visit.

LPA interviewed children. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided to the director.

An exit interview was conducted and a copy of this report was provided to facility staff, appeal rights and notice of site visit issued.

The Notice of Site Visit must be posted for 30 days.
