On 10/17/25 at 11:20 am, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent complaint investigation to deliver final findings. LPA met with director and was granted access to the facility. LPA toured facility and took a census.

Allegations: Lack of supervision resulting in daycare child sustaining injuries. Staff are not properly securing daycare child’s diaper.

It was alleged a child sustained injuries and staff were unaware of the injury. LPA interviewed all pertinent parties, including staff. Staff stated if a child sustains an injury, they will check the child for any visible signs of the injury, and they will write an “ouchie” for the child’s authorized representative. Staff stated at times, children will fall as they are learning to walk. Staff stated they are unaware of a child sustaining an injury without their knowledge

It was alleged staff are not securing a daycare child’s diaper. LPA interviewed all pertinent parties, including staff. Staff stated they do secure children’s diapers. Staff stated sometimes a child may wiggle around when changing a diaper; however, they always ensure the diaper is securely fastened so there are no concerns of diaper rash or other issues with a loose diaper.

Based on interviews, the above allegations are unsubstantiated. Meaning, although the allegations may have happened, or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.

LPA conducted an exit interview with director. LPA provided a copy of this report, appeal rights, and a Notice of Site visit form was issued. Notice of site visit must be posted for 30 days.
