32 | It was reported, on or about 03/09/2022, a child sustained bruising on their mid to lower back while in care. During the investigation, it was disclosed staff conduct health screenings when a child is dropped off and they document their observations. However, staff do not check underneath children’s clothes. During interviews, is was disclosed staff did not observe any bruising on the subject child during diaper changes. Additionally, if a child falls and injures themselves, the Director is notified, and an incident report is created by the staff that observed the incident. During interviews, it was disclosed staff did not observe the child fall or being pushed by another child. Other information received during the investigation, the subject child was described as “rambunctious, plays a lot and falls a lot”, however there were no known injuries reported the day the alleged bruising occurred. Therefore, it is unclear if the child sustained the bruise while in care or elsewhere.
Based on information obtained during this investigation, through interviews conducted, review of pertinent documentation, and after receiving conflicting information, the allegation is UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegation occurred.
An exit interview was conducted with Director Kristen Garnatz, a copy of this report was provided, and a Notice of Site visit was issued.
The Notice of Site Visit (LIC 9213) shall be posted where the parent/guardian of children enter and exit the facility. The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
A copy of this report must be made available for the next three years.
