Licensing Program Analysts (LPAs) Blanca Ruiz and Rachel Zeron arrived at the facility to conduct a visit at the above licensed facility to interview a staff who is currently working at this location. During inspection facility was toured and census was taken. LPAs met with staff and interview was conducted. Facility found in substantial compliance.

No deficiencies cited

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and the report was reviewed with the Director.
