25 | Licensing Program Analyst (LPA) Rachel Zeron conducted a case management inspection with Director, Crystal Reynolds. A case management inspection is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 06/11/2024. LPA received a complaint regarding the same child (C1) on 06/10/2024 which was being investigated at the same time as this incident.
The incident occurred on 06/07/2024, the UIR documented an incident where, while on the playground C1 pushed another child down in the wood chips and began throwing wood chips a the child, child was crying and some wood chips landed in the child's mouth. The incident was self reported and the facility conducted an internal investigation into the matter.
During a previous visit on 06/20/2024, LPA investigated the incident, staff were interviewed and the following was disclosed. There were two staff present and 10 children in care, this was at the end of the day at approximately 5:30pm on 06/10/2024 and classes were being combined. Staff 1 (S1) brought the remaining children out to the playground to join Staff 2's (S2) classroom. As S1 and S2 were checking the children in, the noticed that C1 had pushed down a child into the wood chips, staff ran over. As staff was heading over to the children, C1 began throwing wood chips in the child's mouth. C1 was redirected and both responsible parties were notified upon pick up and given a written incident report. C1 was also put on a behavioral plan and later terminated for continued behavioral issues.
Based on the information obtained, there appeared to be no violations of Title 22 Regulations pertaining to the reported incident at this time.
LPA conducted an exit interview with Director, Crystal Reynolds and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days. |
