25 | Licensing Program Analyst (LPA) Rachel Zeron conducted an unannounced case management visit at the facility for the purpose of concluding an investigation pertaining to the facility's submission of an Unusual Incident Report (UIR) received by Licensing on 07/03/2024, the UIR outlined an incident that occurred on 06/27/2024 pertaining to children left unattended in the classroom . LPA was granted entry to the facility and met with facility Director, Amy Halit and Crystal Reynolds, Director for Chino Hills KinderCare, assisting today.
According to the UIR, on Thursday, 06/27/2024 at approximately 10:39 am during morning recess, two children were found by Staff 3 (S3) in the classroom unattended. S3 indicated that the children were climbing on some toys by the window, while she was standing close by for approximately 5 minutes but did not realize that the staff was not in the classroom. Once S3 realized that the children were alone, S3 entered the classroom and escorted the children out to the playground where S3 met with Staff 1 (S1) and Staff 2 (S2) and handed over the children. S1 and S2 did not know that the children were in the classroom. Staff stated that a child was picked up at approximately 10:33 am by their parent and the children could have followed that parent into the classroom without them knowing. Responsible Parties were informed by the director.
Based on interviews and evidence collected, its has been confirmed that 2 children were left unattended in the classroom. The Facility was found to be in violation of the following Title 22 Regulation:
Responsibility for Providing Care and Supervision: 101229
(a) The licensee shall provide care and supervision as necessary to meet the children's needs.
(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
